Please fill out this form completely or download the PDF version. You will receive an RMA number once this request is processed and approved. DO NOT SEND PRODUCTS IN FOR REPAIR UNTIL YOU RECEIVE AN RMA NUMBER. INTERNATIONAL SHIPMENTS MAY REQUIRE IMPORT OR EXPORT LICENSING, PLEASE CONTACT AEROCOMPTUERS FOR CUSTOMER SPECIFIC INFORMATION. ANY INTERNATIONAL SHIPMENTS TO AEROCOMPUTERS WHICH DO NOT ARRIVE WITH APPROPRIATE EXPORT LICENSING WILL BE REJECTED.

By submitting this form, I acknowledge the following terms and authorize AeroComputers to perform work as necessary. Customer is responsible for shipping costs to send products to AeroComputers. All products should be returned to AeroComputers in their original packaging or other packaging sufficient to ensure safe shipment. AeroComputers is not responsible for product damage incurred in shipping.
1) Warranty. For all 5300s and newer models, for first three years, all hardware repairs/replacements completed at no additional charge. All software updates at no charge for life of unit. For 5100s and earlier models, hardware and software covered for one year. AeroComputers covers costs to return product to customer via FedEx Ground Service. If faster shipping is required, customer must supply AeroComputers with a UPS or FedEx account number.
2) Current Hardware Maintenance Agreement. (For Moving Map Systems on current Extended Hardware Maintenance Contract) All hardware repairs/replacements and completed at no additional charge. Software updates free of charge for units 5300 and newer and $495 for units 5100 and earlier. AeroComputers covers costs to return product to customer via FedEx Ground Service. If faster shipping is required, customer must supply AeroComputers with a UPS or FedEx account number.
3) Without Maintenance Agreement. Customer will be charged a diagnosis fee of $495. Customer will be charged for all applicable parts, labor, and return shipping costs in excess of $495. Customer will be charged $495 for software updates on systems 5100 or earlier. Repair quote will be provided to customer prior to commencement of any repairs or software updates. AeroComputers will return repaired/replaced products to the customer when a purchase order or payment is received.
